Why "Diabetes" Is the Wrong Name

Diabetes mellitus describes glucosuria — an overflow symptom visible only after years of metabolic dysfunction. Energy Storage Disease names the upstream pathophysiology: the four pillars that drive T2DM and can be detected — and reversed — long before the HbA1c threshold.

1

Cellular Energy Lock (Insulin Resistance)

Peripheral and hepatic insulin resistance impairs glucose uptake. Glucose accumulates as overflow, not because of a glucose problem — because the lock is broken. HOMA-IR quantifies this 10–15 years before HbA1c rises.

2

Metabolic Inflexibility

Healthy metabolism switches between glucose oxidation (fed) and fatty acid oxidation (fasted). In ESD, this switch is impaired — resulting in impaired fat mobilization, postprandial hyperglycemia, fatigue, and progressive lipid deposition. Measurable by fasting RQ or TG/HDL ratio.

3

Chronic Energy Surplus Cascade (Twin Cycle)

Caloric overflow exceeds glycogen capacity → visceral fat accumulation → hepatic fat → pancreatic fat. Pancreatic fat drives beta-cell dysfunction. The Twin Cycle Hypothesis (Taylor, 2019) demonstrates this cascade is reversible with sustained energy deficit.

4

Mitochondrial Dysfunction

Reduced mitochondrial density and oxidative phosphorylation capacity impair substrate utilization. mtDNA copy number and oxidative enzyme activity are reduced in skeletal muscle. Exercise-induced mitochondrial biogenesis is a primary reversal mechanism (Mootha 2003, Kelley 2002).

Lab Protocol

Ordering an ESD-Aware Panel

Standard metabolic panels miss ESD because they measure overflow, not the underlying resistance. Add these labs to detect ESD early and track reversal.

Standard Panel — Misses ESD

Fasting glucose Lags 10–15 yr
HbA1c Overflow marker
Total cholesterol Poor ESD marker
Fasting insulin Not ordered
HOMA-IR Not ordered
TG/HDL ratio Not calculated

ESD-Aware Panel — Detects Early

Fasting insulin Optimal < 5 μIU/mL
Fasting glucose Optimal < 90 mg/dL
HOMA-IR (calculated) Optimal < 1.0
TG/HDL ratio Optimal < 1.5
Uric acid Optimal < 5.5 mg/dL
HbA1c (context only) In context of above
HOMA-IR Reference Ranges

Interpreting the Result

HOMA-IR ScoreInterpretationTypical StageRecommended Action
< 1.0Optimal insulin sensitivityHealthy metabolic functionMaintain — repeat in 2–3 yr
1.0 – < 2.0Acceptable — normal rangeLow-riskLifestyle coaching, annual screen
2.0 – < 3.0Early insulin resistanceEarly ESD — subclinicalIntensive lifestyle; 6-mo recheck
3.0 – < 5.0Significant insulin resistanceSignificant ESD — pre-diabetes windowStructured dietary intervention + repeat labs
≥ 5.0Advanced insulin resistanceESD advanced — diabetes risk highIntensive program ± pharmacotherapy + endocrinology

HOMA-IR = (Fasting insulin [μIU/mL] × Fasting glucose [mg/dL]) ÷ 405. Use molar units for glucose if ordering SI labs: (Glucose [mmol/L] × Insulin [mIU/L]) ÷ 22.5

Clinical Evidence

Reversal Is Real — The Data

These are not pilot studies. These are randomized, peer-reviewed trials demonstrating measurable, durable metabolic remission.

46%

DiRECT Trial — Dietary Intervention

46% of intervention participants achieved HbA1c < 6.5% without medication at 12 months. Weight loss of ≥ 10 kg correlated with 57% remission. The intervention: 825–853 kcal/day formula diet for 3–5 months.

Virta Health 2-Year Study — Nutritional Ketosis

60% of participants reversed T2DM (HbA1c < 6.5% without medication) at 2 years. 94% reduced or eliminated insulin. Mean weight loss 12.8% at year 1. The intervention: continuous nutritional ketosis via individualized low-carbohydrate diet with remote physician monitoring.

Twin Cycle Hypothesis — Ectopic Fat Clearance

Sustained caloric restriction (600 kcal/day) cleared hepatic and pancreatic fat within 8 weeks in T2DM subjects, restoring first-phase insulin secretion and normalizing fasting plasma glucose. Provides mechanistic basis for the ESD reversal model.

Mitochondrial Biogenesis in Reversal

Exercise-induced mitochondrial biogenesis (via PGC-1α activation) restores oxidative phosphorylation capacity in insulin-resistant skeletal muscle. Reduced mtDNA copy number and OXPHOS gene expression are consistently observed in T2DM and prediabetes.

Adding ESD Screening to Your Practice

A practical 5-step workflow for integrating HOMA-IR into routine metabolic screening.

1

Order Fasting Insulin at Baseline

Add CPT 83525 (fasting insulin) to your next metabolic panel for patients with any of: BMI > 25, NAFLD, PCOS, family history of T2DM, hypertension, acanthosis nigricans, elevated TG, or HDL < 40 (men) / 50 (women).

2

Calculate HOMA-IR at the Encounter

Use the free HOMA-IR calculator at energystoragedisease.com, or in your EHR: (Fasting insulin × Fasting glucose) ÷ 405. Document as a structured note field. Target < 2.0 for acceptable sensitivity (< 1.0 is optimal).

3

Stage the Patient Using the ESD Framework

HOMA-IR 2.0–<3.0 = Early ESD; 3.0–<5.0 = Significant ESD; ≥5.0 = Advanced ESD. Scores below 2.0 are Optimal or Acceptable. Frame the diagnosis as Energy Storage Disease — patients understand the storage/overflow model better than abstract glucose thresholds. It increases engagement with lifestyle modification.

4

Prescribe the ESD Patient App

Direct patients to the free ESD app (iOS + Android). It includes: the HOMA-IR calculator with up to 10-reading trend history (so patients track progress between appointments), a 6-marker lab explainer they can share with lab, the 4-pillar ESD framework, and reversal evidence summaries. No account required; all data stays encrypted on device.

5

Monitor at 6 Months — Track the Cascade in Reverse

Repeat HOMA-IR + TG/HDL ratio at 6 months. ESD reversal typically follows: TG/HDL improves first (weeks), HOMA-IR improves next (months), then HbA1c (6–18 months). Document the cascade to show patients measurable progress before HbA1c normalizes.
